Liquid biopsy, especially plasma ctDNA, is no longer a concept, it’s part of daily clinical practice across multiple tumor types.

✅Driving treatment decisions with real-time molecular insights
✅Enabling escalation & de-escalation strategies (e.g. CRC)
✅Identifying patients for adjuvant immunotherapy (MIBC)
✅Expanding into early detection, risk stratification & MRD

✅Tumor-informed assays remain dominant for MRD, while new biofluids like CSF are opening additional windows into tumor biology.

⚠️ Challenges persist but so does momentum.

🧬 Toward a multimodal future in lung cancer care

At #ELCC2026, ISLB President Umberto Malapelle highlights how integrating ctDNA, imaging (LDCT), proteomics, and clinical data can refine early detection and risk stratification in NSCLC.

✅ ctDNA + LDCT improved PPV from 62% → 84%
✅ Sensitivity 78.5%, specificity 92.0% in early-stage disease
✅ Persistent ctDNA linked to early relapse risk
✅ Proteomics adds value for long-term prediction

📌 Take-home: no single biomarker is enough multimodal strategies are shaping personalized oncology
