Sadly, it was hard to get this paper published. Apparently, it is hard to find a good home for papers about RCTs with so called “negative findings” (although we do not like this term). This warrants more discussion in the academic community! 10/10.

Many patients valued the ACP conversations, yet, 16% found them distressing. This is something that deserves more attention. 7/10

We did find that specialist palliative care was more likely in the intervention group (37% versus 27%). Also, patients in the intervention group more often had an AD in their hospital file (10% versus 3%). It was striking that these
numbers were rather low, though. 6/10

What did we find? Mean change in patients’ quality of life did not differ between intervention and control participants, nor did changes in symptoms, coping, patient satisfaction, and shared decision-making. 5/10

Who participated? Patients with advanced lung (stage III/IV) or colorectal (stage IV) cancer, WHO performance status 0–3, and at least 3 months life expectancy were eligible. We recruited in 23 hospitals in 6 countries: Belgium, Denmark, Italy, Netherlands, Slovenia, and UK. 4/10

The intervention had multiple components: (1) scripted ACP conversations between patients, families, and certified facilitators; (2) standardised leaflets; and (3)
standardised advance directives especially developed for this study. Intervention fidelity was high, 86%. 3/10

What did we do? This was a large study. In a cluster RCT, we randomized 1,117 patients: 442 intervention and 675 control of whom 809 (72%) completed the 12-week questionnaire. Patients in the intervention group were offered the ACTION Respecting Choices ACP intervention. 2/10
