Although more data is needed, there have been some data to suggest absolute risk is not exceedingly high - of note: a bit younger cohort, with limited # with #CAD - and risk not 0, but not as high as we may think

If they were losing weight and having ongoing symptoms, even if early in IL-23 course, I might either pivot to a faster-onset med or add on as you said :)- best option will be the most quickly efficacious to prevent #frailty & AEs
