🚨 New #VExUS post on https://t.co/hqYw2WlFqR
Confused by abnormal hepatic vein Doppler waveforms? This post breaks them down using physiology, making it easier to connect waveform changes to the underlying hemodynamics.

If a component of VExUS feels difficult, the answer is to understand it better, not to eliminate it altogether. Simplification can be helpful to some extent, but oversimplification often comes at the cost of losing valuable physiological information.
🧵 What drives blood flow – the heart or the vessels?
Eminent physiologists have argued this for decades.
The disagreement survives because of imprecise causality.
Here’s the resolution 👇
Training is central to doctors providing the best care for patients.
The Medical Training Review, out today, is a diagnostic report with input from many doctors, medical students and patients.
It shows we need to rethink many aspects of training.

@jeffreygeske@JAMACardio I was expecting a wide pulse pressure ! B blockers should be used with caution in severe AR. As they could worsen regurgitant fraction although resting HR 100
Ever wondered what determines the hemodynamic impact of tricuspid regurgitation?
What is the impact of RegVol/RegFr on RA V waves and on stroke volume? What the impact of RA compliance on RA V waves?
Can we safely use indirect Fick in tricuspid regurgitation?

When CP is mixed with myopathy, e' is lower than that in pure CP. Constriction until proven otherwise when e' is⬆️ in HF patient!
Obituary for Rob Sapsford (1964-2024): Leeds cardiologist whose breadth of knowledge and sense of humour have left a huge hole.
Sapsford's warmth meant his office became an unofficial break room as colleagues would often pop in for a chat

Of the estimated 20,000 human gene products ~4,400 are thought to modify human disease. But if your protein isn’t one of the ~650 targeted by approved therapies…how do you drug it❓Out today @CellCellPress https://t.co/sWNDYqqbTm
@argulian By definition isolated post cap PH has a normal PVR<2WU. Mid systolic notch indicates raised PVR so it would be combined post& pre capillary PH.
