Based on the scenario shown in the image, if a blood clot is present inside the IV cannula, can forcing it inward via a syringe flush cause a Thrombus or clinical complications for the patient Or No?!
A Med Student asked me during rounds:
"Why is Zinc Gluconate always given in diarrhoeal disease? I don't understand why."
Many people have not really understood the vital mechanism behind it.
@dr_yograj Well done! This is indeed the capsule sponge, potentially a gamechanger for Barrett’s oesophagus surveillance based on the must read DELTA trial, presented in Nottingham today ⭐️🏆

This is the Cytosponge (also known as the “sponge on a string” or Cytosponge-TFF3 test), a minimally invasive procedure for sampling cells from the esophagus.
————————————————
How it works —————
• The patient swallows a small capsule (pill) attached to a thin string. The capsule contains a compressed sponge.
• In the stomach, the capsule coating dissolves, and the sponge expands.
• The sponge is then gently pulled back up through the esophagus by the string. As it moves, it collects cells from the lining of the esophagus.
• The retrieved sponge is sent for lab analysis (often using a biomarker like TFF3) to check for abnormalities.
Main uses: —————-
• Screening for Barrett’s esophagus (a precancerous condition linked to chronic acid reflux/GERD).
• Helping detect or triage esophageal cancer (e.g., squamous cell carcinoma or adenocarcinoma) earlier, especially in higher-risk patients.
• It’s an outpatient or even GP/clinic-based alternative to traditional endoscopy (EGD), quicker, less invasive, and more comfortable for many people.
This matches the context from the X thread (replies mentioning esophageal sponge cytology, Barrett’s screening, pill-on-a-thread, etc.), and Dr. Siau (a GI specialist) confirmed it wasn’t the older Entero-test/string test for parasites like Giardia.
It’s a “hot topic” in gastroenterology for non-endoscopic esophageal screening. The procedure is generally well-tolerated, though some feel mild discomfort when the sponge is pulled out. @IhabFathiSulima #MedTwitter #Meded #MedX @Lap_surgeon
Child-Pugh score estimates hepatic functional reserve and prognosis in cirrhosis using bilirubin, albumin, INR, ascites, and hepatic encephalopathy.
Higher Child-Pugh class indicates more advanced liver dysfunction and poorer prognosis.
