… and the winner is: the Unscheduled Care Coordination Hub 🎉
To date the hub has been able to manage over 3,500 patients, reducing the need for an EMAS call out and providing advice, support and intervention instantly to support patients to remain safely at home.

We're delighted that our Unscheduled Care Coordination Hub has been shortlisted in the #HSJawards , for Provider Collaboration of the Year. 🏆
The Hub assesses patients who've called 999 so that, where appropriate, treatment & support is provided at home rather than hospital.
@AgeofAutism Adult mental health doesn't meet their needs . It's not a learning disability or a mental health problem they fall into this void . My son is 22 now and still relies on me when he has a meltdown .he hasn't made any friends and is very isolated at university.
