On examination, vitals and the rest of the neurological/systemic examination were normal, except for this intermittent abnormal neck posture.
The neck repeatedly goes into hyperextension with deviation to the left, but returns to normal between episodes.

Clonazepam vs Zolpidem: Same Goal, Very Different Sleep Pills

@DocPriyamMD comparing clonazepam and zolpidem for sleep makes an important point: both can be prescribed for insomnia, yet they are not interchangeable. Understanding how they differ in timing, duration, and risks helps patients use them more safely if a doctor decides they are needed.
Clonazepam is a long-acting benzodiazepine. It is usually taken at night and can be taken after dinner; there is no empty-stomach rule. Because it stays in the body for many hours, it may help people stay asleep through the night. The trade-off is residual grogginess the next day. The same drug is also used for panic attacks and seizures, so its effects go beyond simple sedation.
Zolpidem is a shorter-acting “Z-drug” designed more specifically for sleep. It should be taken only when you are getting into bed and have a full 7–8 hours available to sleep. It works faster on an empty stomach; food can delay its onset. While it typically causes less next-day hangover than clonazepam, it has a well-documented (though uncommon) risk of complex sleep behaviors such as sleepwalking or sleep-driving with no memory of the event.
Both medicines require a prescription. Both can cause dependence if used regularly. Mixing either with alcohol is dangerous. They are generally intended for short-term use while the underlying sleep problem is addressed.
Sleep medicines can reset a disrupted clock when insomnia is severe, but they are not a first-line or long-term solution for most people. Lifestyle changes, consistent sleep schedules, and treating the root cause remain the foundation.
My opinion: — For sleep, medicines should always be avoided if possible. They do help in setting the sleep clock right when used carefully and briefly. Clonazepam is a benzodiazepine. It enhances GABA signalling at GABA-A receptors and stays in the body much longer, which can mean drowsiness the next morning. There’s no standard insomnia dose; Many sleep guidance lists 0.5-2 mg/day as a range. Watch for poor coordination, falls, memory problems and dependence. After regular use, stopping abruptly can cause withdrawal seizures.
Zolpidem is a Z-drug. It also acts at GABA-A receptors but favours those containing the alpha-1 subunit and leaves the body faster. For standard tablets, FDA starting doses are 5 mg for women, 5-10 mg for men and 5 mg in older adults, taken just before bed. Dizziness, next-day impairment and dependence can occur. Rarely, someone may sleepwalk or drive without remembering it later; that carries a boxed warning.
Alcohol, opioids and taking the two together increase the risk of dangerous sedation.
Clonazepam and zolpidem can both be prescribed for sleep. But they are very different.

• Lasts longer; may leave you groggy tomorrow
