A 23-year-old man presented with vernal keratoconjunctivitis with steroid-induced glaucoma in both eyes. Intraocular pressure was right eye (OD) 40 mmHg, left eye (OS) 42 mmHg. Significant optic nerve head (ONH) cupping was noted in both eyes (A, OD; B, OS). Topical steroids were immediately stopped and the patient was started on maximum topical antiglaucoma medications with oral acetazolamide and syrup glycerol. Intraocular pressure was controlled on the fourth day of treatment (OD 16 mmHg, OS 18 mmHg) with drastic reversal of ONH cupping (C, OD; D, OS). Unlike children, reversal of ONH cupping is rare in adults due to the reduced pliability and elasticity of lamina cribrosa. These images highlight that although acute intraocular pressure rise can lead to mechanical compression of axons at lamina cribrosa, aggressive control of intraocular pressure and elimination of the primary cause can reverse ONH cupping even in young adults.

While performing a routine cataract surgery, Dr. Sarvess Prakash discovers a mysterious tubular structure attached to the posterior capsule of his adult patient's eye, extending into the vitreous cavity and toward the optic disc.
This structure was determined to be a persistent or intact Cloquet canal, also known as a hyaloid canal, which usually disappears during embryologic development.

A healthy 35-year-old man presented with blurred vision in the left eye 5 hours after landing from air travel. A, Two aberrant macular vessels, a tributary of the superior retinal vein and a branch of the inferior retinal artery, were seen. B, OCT demonstrated intraretinal and subretinal fluid in the superior fovea. C, OCT angiography identified arteriovenous communication between the aberrant vessels. D, Fluorescein angiography, performed 24 hours after symptom onset, confirmed the arteriovenous communication with early filling of the branches of that arteriole and absence of active leakage. The symptoms resolved spontaneously within 3 days. Transient rise in hydrostatic pressure at high altitude within the arteriovenous communication was likely the cause of the leakage.
From “Congenital Arteriovenous Communication Accompanied by Macular Edema after Air Travel” by Ta-Hsiang Wong, MD and Chih-Heng Hung, MD. Published by Ophthalmology Retina online on April 22, 2026.

Let’s understand the hierarchy so that people understand what should be done at what stage.
A. Standard Textbooks (eg Robbins)
B. Main notes of platforms like Marrow/Prepladder
C. Rapid Revision notes of those platforms or BTR or Medsynapse.
Main notes are concise version of Standard textbooks - not replacement.
Read standard textbook (of at least important topics! in MBBS 2/3/4th year for proper good understanding and use Main notes as supplements for Professional exams.
Main notes of all 19 subjects also however by themselves are not revisable for NEET PG when you start preparing for it in internship. They are 10000 ++ pages.
In comes - revision notes.
Revision notes are to be used if already done standard textbook + main notes in MBBS.
Add PYQs to it and you’re good to go.
RR + PYQ.
Use First Aid instead of Revision notes for 1/2nd prof subjects- you will be much more confident.
Now, what if you didn’t do standard textbook or main notes in MBBS?
Should you do that (standard textbook) or main notes of all 19 subjects kn internship now.
No. You don’t have time. That ship has unfortunately sailed. Too bulky and too low yield for effort invested.
If you want a top 5k rank - it’s still doable in INTERNSHIP - but now needs strategic approach.
50% syllables is Path/Pharma/Micro/Obg/ Sx
Do Main notes once —-> with simultaneous concising into First Aid instead.
You read once for concept but simultaneously concise into FA or make own small notes since you won’t be able to revise.
These 5 subjects.
Rest RR + PYQ and use videos if not understanding something.
Tryung to do Robbins as intern while I commend the effort is - foolishness.
Trying to do FA in 2nd year itself - while I commend the effort - again is foolishness.
Hope this clears everything out.
All the best to anyone preparing for next year.
By now, most people would have seen this video & know who Tinkle Gongo from Itanagar is.
Almost every media outlet has covered her story.
But her spirited pushback against an insult & her confident assertion-“Hum bhi India hai”-are reason enough for her to still be my inspiration for starting the week off well.
Thank you, Tinkle.
You are the essence of India…
