We can’t extrapolate adult studies to children as connective tissue layers are different and spread may be different (which needs to be studied). Clinically, it’s a good, safe alternative to epidural. Epidural still remains gold std.

First dose is around 0.2-0.3 ml/kg of 0.125% levo bupivacaine/ 0.2% ropivacaine. And later we run infusions of same concentration at 0.1-0.2 ml/ hour (roughly 0.2 mg/kg). We have tried a couple of cases with intermittent boluses every 4-6 hours.

Most of the times coagulation is deranged in these patients which limits the use of central neuraxis (Epidural catheters). So its far safer to use this block.
