Why Nitroglycerin first?

In acute cardiogenic pulmonary edema, the immediate problem is elevated preload and pulmonary capillary pressure, causing fluid to flood the alveoli and severe dyspnea.

Nitroglycerin acts within minutes by:

Venodilation → ↓ preload

↓ Pulmonary capillary pressure

Rapid relief of pulmonary congestion and dyspnea

This gives immediate hemodynamic improvement.

Why not Furosemide first?

Furosemide works by diuresis, which takes 20–30 minutes (IV) to significantly reduce fluid volume.
It is useful after stabilization, but it does not relieve pulmonary congestion as rapidly as nitrates.

Clinical sequence (typical emergency approach)

1. Oxygen / NIV

2. IV Nitroglycerin → rapid preload reduction

3. IV Furosemide → remove excess fluid
