Sorry to say this, but every #caste in #Maharashtra has become an independent nation which has hostile or no diplomatic relations with other nations (i.e. castes). Our politicians and intelligentsia have let down our society. #History will never forgive their role in this sordid episode.

We have a new paper published on something very important and I would like to simplify it for you. But before that, you have to understand something bittersweet about this strange but devastating, not so uncommon disease.
A few years ago, two young women came to my OPD and told me that their father was not "a mad person" like everyone else was claiming. Over the course of few months, he was home-bound, then bed-bound and people around him finally realized that he had gone "full cuckoo" and let him be. Except his daughters. They knew their father so well, to understand that there was no way he could behave like a bipolar-maniac like the psychiatrists they were showing him to claimed. According to them, he was a sweet, brilliant man, and he could not have a psychiatric disorder in a years time that incapaciated him. I believed them, and took his case up.
This was not an isolated incident. A young graduate from Bangalore visited me because his father was unable to make calculations and kept forgetting things around the house and would burst into anger spells with the mother and son. Neurologists and psychiatrists diagnosed him with dementia disorder with psychotic symptoms. He worsened rapidly. He was a maths teacher at a prominent university and there was no way he could get his calculations wrong.
I believed the son and took his case up.
Another man, a son, rushed his father into the emergency one day. He was behaving strangely and he defecated in front of his family on the dinner table. He suddenly became abusive and then went into a state of confusion, progressing to coma. Elsewhere, they told him it was a stroke and he could not get better. But the son could not believe it. I believed him, and took his case up.
In 2020, a group of researchers across the world, saw many children with autistic symptoms, managed as autism, and various psychiatric illnesses - depression, anxiety, hyperactive disorders. But their parents believed that there was something more to it and the diagnosis of autism in their children was more than meets the eye. So they took them to various doctors, finally ending with liver specialists. The parents believed that their children were not autistic. The doctors believed them and took their cases up.
All these patients had one common disease. It was called portosystemic shunt syndrome or PSS - a condition that causes blood from the portal circulation (intestines, spleen and liver) to bypass the liver and flow directly into the systemic circulation, through a large, abnormally developed vein, called as shunt, leading to high ammonia levels in the body, that affects brain functions and lead to a host of neuro-psychiatric symptoms leading to various misdiagnoses.
It may or may not be associated with cirrhosis. In the group of children with "supposed" autistic disorder, there was no cirrhosis and it was a congential disease (from birth) - called Abernathy Malformation.
https://t.co/vNbZZ2fyqB
The treatment is simple. Close/block that damn shunt - through a simple, angiographic/radiological procedure called "shunt embolization". And we did.
The two daughters got their father back. He was not a "looney" anymore. They went for family trips together and attended weddings of their cousins. The father of the young man from Bangalore went back to getting his calculations correct. He was never angry. The man who defecated in front of his family and came in a coma to my ER, went back home walking after two weeks. He was never told what he went through, but his family is still trying to forget that which they witnessed [https://t.co/b6yUDPgE6x].
And all the kids, diagnosed with "autism" and "psychiatric diseases" woke up the next day as different little people, seeing the world differently. They were not autistic anymore. This is what timely diagnosis and shunt block does, in this dreadful disease...
- a reminder to doctors: "The good physician treats the disease; the great physician treats the patient who has the disease." - Sir William Osler.
Our unit, arguably performs the maximum number of shunt blocks in India, which is life changing for many patients. And @SRajesh_IR is the expert in it. We also have the largest number of publications on this from the country.
We did the largest series first [https://t.co/9xS6YOsMSY], followed by a larger series on early vs late shunt block [https://t.co/jS1IBV9qEw] and now we have this new paper that shows how to predict clinical outcomes in those undergoing shunt block in cirrhosis based on liver pressure variables - something very novel and new in medical literature.
Thank you for reading. Please share this so that other (especially doctors) will know about this disease entity that hides in plain sight.

I was interviewed by @janeborges9 for @mid_day regarding science-based medicine and Ayush practices in the wake of my recent suspension from Twitter (X).
I have few points to clarify about this report and my interview.
The final report has become a "Brandolini's Law" kind of situation for me.
"The Bullshit Asymmetry Principle, also known as BrandoliniтАЩs Law, states that the amount of energy needed to refute bullshit is an order of magnitude bigger than that needed to produce it."
I was dissapointed to see that the feature included multiple Ayurveda and Homeopathy practitioners who have very conviniently, used the medium to burden the readers with more misinformation on diagnosis and treatments for various diseases.
I am not sure why there is even a debate on this - scientific medicine vs. pseudoscientific medicine.
There is no debate. It is plain and clear that scientific medicine is the closest we come to truths in healthcare and pseudoscience like Ayush is a distraction that is fuelled by weak anecdotes, appeal to tradition, culture and antiquity and embraces the business of healthcare and not art of patient care.
It is like inviting Hamas or Hezbollah or ISIS terrorists to the United Nations and asking their view points on world peace. It is absurd.
Health reporters, please do not give undeserving credit or a seat at the table, to pseudoscience peddlers, in a serious interview or healthcare report, if you really wish to inform the public on right to healthcare.
One of the Ayurveda practitioner interviewed was only looking to sell their quackery business (Gynoveda) through this interview, while another white-washes Ayurvedic pseudoscience with anecdotes and personal experiences.
The Homeopath has no idea what is being discussed. She goes on to say, "People are prescribing ridiculous dosages without following the set principles of homeopathyтАЭ
- I dont get this. Homeopathy is itself so ridiculous and ridiculous dosing means either ridiculous amount of water, sugar or alcohol. Such instances go without rebuttals in this interview. I do not even know why mainstream media must interview Homeopaths regarding anything remotely related to healthcare. They dont know anything about human anatomy, physiology, biochemistry and pharmacology. They still believe in magic.
@mid_day has also supported chemophobia (fearmongering general public on "synthetic chemicals") through this article.
The New-York based Vaidya (not doctor), Nidhi Pandya blatantly says, "in the last 10 years alone, the US Food and Drug Administration (FDA), which is responsible for protecting public health by ensuring the safety, efficacy and security of human and veterinary drugs, has had more than 14,000 drug recalls. This means that they were initially considered тАШsafeтАЩ for use, before being recalled. Ayurveda is a 5,000-year-old science, which is also time-tested. And while there may be concerns about who is using it, how itтАЩs being administered and processed, weтАЩve never really seen a case, in all these years, of a particular substance thatтАЩs deemed to be absolutely unsafe to the point that itтАЩs banned.тАЭ
Really?
Did this report even go into details of this claim on no ban on Ayurvedic supplements?
[1] European Union ban on Ayurvedic Medicines - https://t.co/fLfR6lhLj4 & https://t.co/gyj3jbssvu
[2] Banned -- Select: "a drug to alter the sex of the foetus" - https://t.co/9SLIHOJ8Rg
[3] Canada bans ayurvedic drugs - https://t.co/cFzgSbTZUX
[4] Singapore bans ayurveda products from India, UK alerts public - https://t.co/EgRpA6LMlI
[5] Why did Denmark ban Ashwagandha? - https://t.co/I7o2c8E8uN

[7] тАШAyurvedicтАЩ pills banned after steroids found in them - https://t.co/Ol3iKtVo1i
- Drug recalls in scientific medicine, happen as part of phase IV clinical trials for safeguarding public health and patient safety. It is one of the most important aspects of scientific medicine and showcases maturity of the science-based practice, and is not a weakness. Ayurveda has no clinical trials to back up efficacy or safety and the largest upcoming cause for acute liver injury in the West is herbal drugs while the commonest cause of drug-induced liver failure in cirrhosis patients in East is herbals. None of these clarifications are provided in the report.
The Ayurveds, driven by cognitive failure keeps talking about how they diagnose conditions "holistically" through Vata, Pita, Kapha
- I mean a newspaper putting statements on obsolete humoral theories in 2023 shows how regressive this country still is, when it comes to scientific temperament.
The Homeopath is just delusional in this interview. She says, "This alternative therapy, she believes, can be used to treat everything from a common cold to cancers (of earlier stages). But ER physicians are recommended for emergencies and surgery-related cases."
- this is called cognitive dissonance 101.
Cognitive dissonance is "a mental conflict that occurs when your beliefs don't line up with your actions. It's an uncomfortable state of mind when someone has contradictory values, attitudes, or perspectives about the same thing."
Even after repeated and ongoing attempts at telling people and especially the media, not to use the term Allopathy for scientific medicine, it features full on in the title. Health reporters, please do not use this term again. In this interview I have already discussed what Allopathy is and it still featured in the headline, erroneously synonymous with science-based medicine.
I will leave it to the reader (you) "to separate the wheat from the chaff" in this very chaotic report.
