Very interesting press release from @Merck this morning indicating that the #KN756 met its primary endpoint of improving pCR (pathological complete response) in patients with high-risk, early-stage HR+/HER2- #breastcancer

I am cautiously excited about this press release until I hear more because:

1️⃣We know that in HR+ (unlike in TNBC and HER2+) that not archiving a pCR does not always translate in poorer outcomes and so it is really important to analyze the EFS (Event-free survival; the co-primary endpoint) data in both the IO and SOC groups before making any definitive decision

2️⃣How tolerable/toxic is the addition of immunotherapy to SOC

3️⃣Which patients with HR+/HER2- truly benefit from the addition of IO?
(ie did all the patients in the eligibility criteria benefit or only a subset?)

Finally, It is a shame they did not include #lobular Histology for the following reasons:

1️⃣ In the KN28, 2/3 of responders to immunotherapy had #Lobular histology

2️⃣ The Gelato trial showed that immunotherapy led to so responses in #ILC

3️⃣ We have recently shown in our poster at #ASCO23 that 44% of #ILC are immune enriched (Abstract 579; https://t.co/hQ3FxKuoTI ; @BostonGeneCorp)

Happy to share our @MDAndersonNews newly published paper about #lobular_breast_cancer in the European Journal of Cancer

Our study looked at the prognostic value of LCIS (Lobular Carcinoma In Situ), a facultative precursor of #ILC, when found at the time of diagnosis of #ILC

3 Key points from the study:

1️⃣ LCIS co-occurs with ipsilateral #ILC in 45% of the cases at the time of diagnosis

2️⃣ Pure #ILC tend to be associated with higher stages at diagnosis compared to ILC+LCIS

3️⃣ Absence of LCIS at the time of diagnosis of #ILC is a poor prognostic factor

IMO (In accordance with our #ILC pathologist @LP_Middleton) 👉🏻 All pathology reports should indicate whether LCIS is present or absent when reporting a diagnosis of #ILC
