RFK Jr. has spasmodic dysphonia, a rare neurological voice disorder (focal laryngeal dystonia) with symptoms he first noticed around 1996 at age 42. He has said he suspects a possible link to the flu shots he was getting annually, after seeing mentions of it while reviewing vaccine materials for litigation. He stopped flu shots in 2005 and calls it a potential culprit he cannot prove or rule out.
There is no scientific evidence that flu vaccines cause spasmodic dysphonia. Experts and the Dystonia Medical Research Foundation state no known link. It is not listed as a side effect in current flu vaccine package inserts (one older listing of general "dysphonia" came after he stopped). It is a specific condition, not generic, with unknown causes though sometimes tied to other triggers.
