Do all dietary emulsifiers have the same impact on the gut❓

✅ An emulsifier-free diet may ⬇ cholesterol levels
✅ Following an emulsifier-free diet + 5 different dietary emulsifiers (one each time):
🔸No changes in gut microbiota diversity
🔸⬇ Short-chain fatty acids, especially in people who consume the food additive carboxymethyl cellulose
🔸No differences in:
👉 Intestinal inflammation (faecal calprotectin, C-reactive protein)
👉 Cholesterol levels
🔸Carragenan ⬆ permeability
💡 The impact of dietary emulsifiers on gut microbiota, intestinal inflammation, and permeability seems to be dependent on the emulsifier
As one food contains more than one emulsifier, it seems prudent to decrease the consumption of foods rich in emulsifiers for better gut health

@IldikoMe This is such an interesting time. Many of the children I worked with are now through college, moving out of pediatrician GI offices, and moving into adult practices. All use diet w no to varying degrees of meds. These practices are seeing long term remission paradigm change😯

I’ll give you a rare personal n of 1 to make a really important point because the kids, teens, and young adults who struggle are why I continue to be here.
When her GI asked at a checkup this week how she would rank her therapeutic diet of 7+ years on a scale of 1 to 10, with 1 being ‘most disruptive and difficult’, and 10 being ‘not at all disruptive or difficult’, she gave it an “8 or 8.5”.
Perhaps it’s time to start listening to patients and parents about how to improve quality of life and outcomes via dietary therapies. Be inquisitive. Ask patients who are doing the best. We are the experts here.
Diet and lifestyle (sleep, exercise) are also critical pieces of psychological wellbeing. Teaching teens they have agency and that they are not chronically ill victims creates empathetic, mature, and resilient individuals.
Nothing improves quality of life more than early effective and persistent disease control. By focusing exclusively on immune-directed drugs, the incontrovertible evidence is that you all but guarantee that the vast majority of kids, teens, and young adults will live with active disease and symptoms, at least on and off, leading to a plethora of psychological struggles as listed below.
The current treatment paradigm serves absolutely everyone, except patients.

Muscle and bone are closely linked.
Age-related loss of muscle (sarcopenia) and bone (osteoporosis) are both independently related to loss of physical function and independence into older age.
Great review by @csmithAEP and colleagues showcasing the many links between muscle and bone and the need for treatment strategies to target both of these important tissues.

So, I spoke to people getting food at a food bank and here are some things I learned from those in need:
1. Everyone donates Kraft Mac and Cheese in the box. They can rarely use it because it needs milk and butter which is hard to get from regular food banks.
2. Boxed milk is a treasure, as kids need it for cereal which they also get a lot of.
3. Everyone donates pasta sauce and spaghetti noodles.
4. They cannot eat all the awesome canned veggies and soup unless you put a can opener in too or buy pop tops.
5. Oil is a luxury but needed for Rice a-Roni which they also get a lot of.
6. Spices or salt and pepper would be a real Christmas gift.
7. Tea bags and coffee make them feel like you care.
8. Sugar and flour are treats.
9. They fawn over fresh produce donated by farmers and grocery stores.
10. Seeds are cool in Spring and Summer because growing can be easy for some.
11. They rarely get fresh meat.
12. Tuna and crackers make a good lunch.
13. Hamburger Helper goes nowhere without ground beef.
14. They get lots of peanut butter and jelly but usually not sandwich bread.
15. Butter or margarine is nice too.
16. Eggs are a real commodity.
17. Cake mix and frosting makes it possible to make a child’s birthday cake.
18. Dishwashing detergent is very expensive and is always appreciated.
19. Feminine hygiene products are a luxury and women will cry over that.
20. Everyone loves Stove Top Stuffing.
In all the years I have donated food at the Holidays, I bought what I thought they wanted, but have never asked. I am glad I did. If you are helping a Family this Christmas, maybe this can help you tailor it more. It does for me!
