Patients with #sicca syndrome who have #salivary anti-#Ro60, but no serum autoantibody, constitute a new phenotype of #Sjögren’s disease (SjD) that exhibit clinical objective examinations of dryness that are nearly identical to those of subjects with SjD. This phenotype may correspond to an early-onset SjD stage or belong to group that will remain seronegative while being saliva positive for anti-Ro60.

These findings support the premise that in some patients with sicca syndrome and SjD, salivary glands are sites of antibody production and that these antibodies travel from the salivary glands to the oral cavity, and may offer an alternative and potentially more sensitive diagnostic tool than traditional markers like focus scores.

Wong-type #dermatomyositis (DM) is a rare variant (~46 published cases) with clinical and pathological overlap between DM and #pityriasis rubra pilaris (PRP). In addition to other typical DM rashes, patients with Wong-type DM can present with #psoriasiform plaques overlying the extensor surfaces of the extremities. Skin biopsy of this psoriasiform dermatitis shows confluent parakeratotic and orthokeratotic scale overlying in a checkerboard pattern without epidermal atrophy or interface change, consistent with PRP.

Wong-type DM has been reported in children and adults with unclear consensus on whether this is an overlap syndrome of DM and PRP or a unique presentation of an already protean disease. Wong-type DM cases published in the literature do not show a connection with specific type myositis-specific autoantibodies or a higher risk of systemic involvement, though cases may still vary in severity.

The #Sjögren's Foundation has developed an evidence-based clinical practice guidelines for peripheral #nervous system (PNS) manifestations in patients with Sjögren’s disease. The objective of this work was to bring #rheumatologists and #neurologists and together to understand, define and align nomenclature used by specialists in connection with PNS involvement in Sjögren's, but could also be useful for other systemic #autoimmune diseases.
