Post-acute sequelae of COVID in children: Pulmonary assessment using impulse oscillometry and the effect of vaccination

➡️In a Taiwanese prospective observational study(July–September 2022) researchers evaluated 209 children aged 6–18 years (median age 10) diagnosed with post-acute sequelae of SARSCoV2 infection (PASC/longC0VID),
➡️Pulmonary function was assessed using both standard spirometry and the more sensitive impulse oscillometer (IOS), which detects small-airway changes effectively in children,
➡️IOS abnormalities were found in 74.6% of participants: 62.9% had small airway disease (SAD) and 12.0% had obstructive lung disease (OLD),
➡️SAD was more common in younger children,
➡️Symptom patterns differed by lung dysfunction type:
- OLD linked to higher fatigue and dyspnea,
- SAD linked to chest pain and cough,
➡️Overall, fatigue (44.8%), chest pain (41.8%), limited activity, and dyspnea were prevalent,
➡️78.7% of children were vaccinated,
➡️Vaccinated participants showed significantly better IOS results, with lower respiratory resistance (R5 and R20) and improved reactance (X5), indicating less airway obstruction and better lung mechanics,
➡️Vaccination was associated with reduced odds of decreased appetite and sleep disturbance but did not significantly reduce core respiratory symptoms (cough, dyspnea, etc.),
➡️The study concludes that IOS is valuable for detecting subtle pulmonary impairment in paediatric PASC and that COVID-19 vaccination appears protective for lung function, though it does not eliminate all sequelae.,
➡️Study does not examine reinfections.
‼️So, AGAIN, even a single SARSCoV2 infection can leave nearly three-quarters of symptomatic children with measurable small-airway or obstructive lung disease detectable only by sensitive testing, proving that “mild” paediatric COVID is not always harmless or transient. When the cumulative impact of reinfections is factored in, each additional exposure risks layering further inflammation, fibrosis, and functional decline onto already compromised airways. This trajectory points toward a generation facing rising chronic respiratory morbidity, reduced exercise capacity, and lifelong health costs unless prevention, especially vaccination and infection avoidance, is prioritized aggressively. The data make clear that dismissing reinfections as inconsequential is medically indefensible!
