ASCENT-03 biomarker analysis at #ASCO26: sacituzumab govitecan showed longer PFS vs chemotherapy in previously untreated advanced TNBC patients who were not candidates for PD-(L)1 inhibitors.
The benefit was observed across Trop-2 expression levels, tBRCA wild-type and mutated tumors, and HER2 IHC 0/HER2-low subgroups.
These exploratory data reinforce SG as an active 1L option in this population, with no clear biomarker-defined subgroup limiting benefit so far.

Giredestrant + everolimus prolonged PFS2 and chemotherapy-free survival vs SOC endocrine therapy + everolimus, with a favorable interim OS trend. 📈
My takeaway: keep testing. ESR1 status still helps interpret benefit and guide sequencing among multiple post-CDK4/6i options. But SERD combinations should not be viewed as ESR1-only strategies.

A few takeaways from the Highlights session: EV/pembrolizumab has reshaped 1L metastatic disease, ADC resistance is the next major challenge, and ADC combinations are rapidly moving into perioperative MIBC.
