Like cant you dissern that it simply might be the Camera Angle and Positioning: The one person you have picked on (WHY NOT EXACTLY THIS AMOUNT OF BLOOD ON HIS HANDS, I CAN SEE THIS IN 4K AND HIS HANDS A CLEAN).
If its the first responder who kneels nearest Kirk's left side (the exit wound side). In some BLURRY cited phone video (filmed from 20-30 yards away in the crowd), i remember it looked like his hands are positioned low gripping Kirk's elbow or shoulder to stabilize him while the main blood flow is downward along Kirk's back, arm, and onto the ground/chair behind him. They removed him pretty quickly tou cannot see who is doing what.

From the first moment, the blood spray arcs forward and left initially but then it would have pooled mostly out of view under his body or on the fabric chair. The helper's palms/face toward the camera show minimal splatter because maybe they're not directly pressing the wound (which i looked up and found could aerosolize blood further); so maybe instead, they're supporting his limbs figuring out how they are going to lift him.
