I might be completely wrong, but based on this photo, my diagnosis is Type 3 polyglandular autoimmune syndrome. This is because if you zoom in and look:
1. The patient’s right foot suggests a diagnosis of vitiligo.
2. The doctor @DrOribaDan is examining the left foot for clonus. The hype from surrounding med students suggests this is positive.
3. Vitiligo + positive clonus is suggestive of subacute combined degeneration of the spinal cord (SCDSC). This happens with severe B12 deficiency, often in the setting of pernicious anaemia. Alcohol can cause low B12, but rarely low enough to cause SCDSC.
4. The combination of vitiligo and pernicious anaemia is seen in autoimmune polyglandular syndrome, specifically type 3.
5. Worth checking cortisol levels, coeliac serology, intrinsic factor and antiparietal antibodies, and administering intramuscular B12, in addition to brain a spinal imaging for completeness.
🏥 35y/F recently started on dialysis and other Rx presents with a skin rash.
She had oral sores and odema. Urine had bld & protein ++++
