🚨 𝗧𝗿𝗶𝗰𝘂𝘀𝗽𝗶𝗱 𝗧𝗘𝗘𝗥 𝗷𝘂𝘀𝘁 𝗺𝗼𝘃𝗲𝗱 𝗵𝗮𝗿𝗱 𝗼𝘂𝘁𝗰𝗼𝗺𝗲𝘀.

TRIC-I-HF (@NEJM, #ESCCongress): 360 high-risk patients with severe TR were randomised to transcatheter repair plus medical therapy or medical therapy alone.

Freedom from death or HF hospitalisation at 3 years was 52.4% vs 21.0% (HR 0.40).

TRILUMINATE and Tri-Fr showed quality-of-life gains. TRIC-I-HF is the first randomised trial with fewer deaths and admissions.

The harder test comes next:
Can everyday cath labs match expert German centres⁉️
Does the benefit hold in earlier disease⁉️

4/ The pattern: TRILUMINATE, https://t.co/XPKMEbwA3b and TRISCEND II made patients feel better. TRIC-I-HF is the first to show they live longer and stay out of hospital. Next question: does it hold in less expert centres and in earlier disease?

3/ The effect: win ratio 2.42 at 1 year. The NNT to prevent one death or HF hospitalisation was about 4. Survival at 3 years was 72.3% vs 53.9%. KCCQ improved by 11.2 vs 4.3 points.

2/ The procedure: 98% of patients got T-TEER, mostly PASCAL (66%) and TriClip (33%). 62% needed two devices, which reflects how anatomically complex these valves are.

1/ Who was enrolled: mean age 80, 56% women, 75% NYHA III–IV, 55% with a recent HF admission, and more than 80% with cardiorenal or cardiohepatic syndrome. These were late-stage patients.

🚨Today’s TCT Plus Middle East program sends a clear signal: intracoronary imaging is moving from optional add-on to procedural infrastructure.

Two live cases put OCT-guided calcified PCI and IVUS-guided left-main PCI in focus, alongside low-contrast PCI and AI-assisted OCT.

Live cases are not outcomes evidence, but they show where workflow is heading: from angiography alone to image-informed planning, execution, and verification, especially in complex anatomy.

The challenge now is routine, consistent adoption across cath labs.

Across 7 RCTs, Odeh et al. showed that:

🔴The composite of Ischemic stroke/systemic embolism was higher with pLAAO than OAC (3.3% vs 2.0%; RR 1.48).

🔴For ischemic stroke alone, #Bayesian analysis failed to establish #noninferiority. Major #bleeding was unchanged.

CHAMPION-AF strongly influenced these ischemic estimates, but the findings challenge broad substitution of pLAAO for OAC.
