On Sept 19, 2025, FDA approved pembrolizumab + berahyaluronidase alfa-pmph (Keytruda Qlex) as a subcutaneous (SC) injection for all approved IV Keytruda solid tumor indications (adults & kids ≥12 yrs) .

📊 PK comparability ✅ (AUC & C trough within 0.8 margin)
🎯 ORR: 45% (SC) vs 42% (IV)
🕒 PFS & OS: no notable differences

💊 Dosing

•790 mg Q6W (± 9,600U enzyme)

⚠️ Warnings: immune AEs, infusion-related rxns, HSCT complications, embryo-fetal toxicity

🧾 JCO Oncol Pract | Thomas et al.

🔑 Key Recs:

1️⃣ Antidotes
 • Dexrazoxane → Anthracyclines
 • Hyaluronidase → Vinca, Paclitaxel/Docetaxel
 • DMSO → Mitomycin, Mitoxantrone (or anthracyclines if no dexrazoxane)
 • Sodium thiosulfate → Cisplatin (>0.5 mg/mL, >20 mL), Bendamustine

2️⃣ Thermal compresses
 • ❄ Cold → Anthracyclines, alkylators (except oxaliplatin), antimetabolites
 • 🔥 Warm → Vinca, etoposide, oxaliplatin, taxanes (+hyaluronidase)
 • ⏱ Longer duration (>24h) preferred

3️⃣ Central line extravasation → early surgical referral/escalation

💡 Goal: tissue preservation, ↓ necrosis, avoid hospital/surgery, prevent chemo delays.
