Cytokeratin can be picked up by mast cells in a sentinel lymph node, potentially leading to a false-positive diagnosis, which is what happened in this case.

Clues to the correct diagnosis include:

1. Subtle but distinct granularity of the cytoplasm

2. Lack of membranous staining which is often present with epithelial cells

3. Nuclear size comparable to adjacent lymphocytes

4. Different morphology from the primary tumor in the breast

As you know, none of these clues is universally applicable and exceptions will arise, particularly with lobular cancer. The most important point is to consider the possibility of a false positive and add a second immunohistochemical stain such as GATA-3 or ER for confirmation.

Also remember that this phenomenon also happens with plasma cells and dendritic cells, though the morphology of dendritic cells is usually quite distinctive and less amenable to misdiagnosis.
