Thousands of people like Margaret can receive hospital-level care at home through virtual wards. Patients are cared for and monitored remotely, and supported by visits from specialist staff when necessary.
This #proactivecare framework has been a long time coming and is more important than ever to help people with complex needs stay well and maintain independence. Definitely worth taking a look!
