Talking to independent physicians, it's obvious that the big insurance carriers are doing to them, what their PBMs are doing to independent pharmacies.
They deny, underpay, slow pay, clawback, and create administrative mazes, knowing their victims don't have the time or resources to fight.
Why ? By putting financial pressures on physicians and pharmacies, it makes them more likely to sell their businesses to them , close their doors, or refer the business to their captive pharmacy or provider. All benefitting the biggest insurance companies
We need to ditch the concept of "claims" and make every delivery of medications or care as a billable event that must, by law, be paid on a timely basis , with interest charges for any delays. If the physician or pharmacy doesn't deliver , the carrier has plenty of legal options already. As does the patient.
This is not an efficient market. This is the big guy abusing the little guy. It needs to change to better the care we get in this country
Yesterday, during a Senate HELP Committee hearing on lowering healthcare costs, @SenTimKaine pointed to #PBMReform as an achievable, bipartisan win that is ready for the taking. Enacting these already agreed-upon reforms would deliver meaningful relief for patients and pharmacies.
Thank you, Senator Kaine, for your leadership on this issue, working to ensure we can rein in the harmful practices of PBMs, realizing lower out-of-pocket costs for patients and helping to safeguard access to the trusted, local pharmacies on which they rely.

Yesterday, I traveled to Arkansas, the state that is the first to take the life-saving movement toward breaking up the health insurance and healthcare monopolies. "The bill will ban PBMs (Optum RX, CVS Caremark, & Express Scripts) from obtaining a license to own their own pharmacies in Arkansas. It will also force PBMs that currently own their own pharmacies here to sell those pharmacies by Jan. 26, 2026. It is a clear conflict of interest that the PBMs — which set reimbursement rates on drug costs for all pharmacies and determine which drugs a health plan will cover — should also own their own pharmacies. Time and time again, PBMs use this unfair advantage to generate a windfall of immense profit for themselves at the expense of patients. They steer patients to their own pharmacies — and they pay their own pharmacies a better reimbursement rate on drug costs than they do their local, independent competitors.
The result: higher drug prices for all patients, fewer choices of where patients can obtain those drugs, and worse quality of care — as local pharmacies close."

“Yet the PBMs are seeking, quite frankly, right now, sky-high profits and health care costs are becoming prohibitively expensive for the average American.
“According to the Federal Trade Commission, three PBMs now control 80% of the market.
“They vertically integrated with suppliers of goods and services, retail, mail order, specialty pharmacies, and large health insurers.
“And as a result of vertical integration, there are pharmacies owned by the same company as the PBM.”
