#COVID2019 | The Papa Giovanni XXIII Hospital in Bergamo, one of the most affected cities in Italy, is already a model for the #coronavirus epidemic management. We share this message from @MicheleSenni, Director of the Cardiology Unit, who turns out to be positive for #COVIDー19.
29 year-old African man, dyspnea and fatigue. 3 years since he has been taking drugs for arterial hypertension, no recent infections, moderate alcol intake, normal coronary angiograohy. LGE pattern? Diff. diagnosis? @Giulia_Vinco@MarziaRigolli@Arzanauskaite@fabrizioricci
@fabrizioricci@Giulia_Vinco@MarziaRigolli@Arzanauskaite@PezelT@Sarah_Moharem We went for the diagnosis of hemopericardium with a pericardial clot. Sadly no real-time free breathing performed, we agreed for the pericarditis, not sure about constrictive physiology (maybe too early with the timing of the trauma?). Follow up echo and CMR planned.
@MarziaRigolli Actually no suspicious echo findings (no respirophasic ventricular septal shift, no increased mitral E velocity, E/A <1, no exaggerated medial mitral annulus early diastolic velocity). Pulmonary vascular congestion on chest X ray, good diuretic response. F-Up echo and CMR planned
Congenital absence of the pericardium:
- CXR: leftward shift of the cardiac silhouette, Snoopy sign, no tracheal deviation
- CMR: left posterolateral rotation and displacement of the heart, apex located in a posterior position
