I was an ANP before medical school and am now a specialty doctor in anaesthetics.

I work with 2 qualified AAs who are excellent,

I am often asked for my opinion given my background, so here it is a long thread including my own story.

@Azeem_Majeed @PJWang12 The issue is, training is removing easy patients, easy procedures as well - how are you suppose to identify / review only complex patients without the background of simpler patients.

@Beska Are you not able to use this evidence to reduce the number of GIM shifts you have to do in subsequent years? Ie if you show that you have done say 9 months in ST4-ST5? (🙏)

-36% ST7s had provisional colonoscopy sign off
-69% interested in hepatology still wanted colonoscopy sign off
-10% felt they could achieve CCT sign off in 4 yrs

If you asked me about greatest change over the 40 years I worked in the NHS I would say the loss of consultant autonomy. The horrifying story of the baby-murders at Chester and the management’s refusal to listen to the doctors is a ghastly culmination of this process.
