This cohort study examined overtreatment trends for men with limited life expectancy (LE) and clinically localized #ProstateCancer in the Veterans Affairs health system from 2000 to 2019. It found that overtreatment, despite increased use of active surveillance, persists for men with intermediate- and high-risk prostate cancer, particularly through the use of radiotherapy. For men with LE less than 10 years, definitive treatment for low-risk disease declined, while treatment for intermediate-risk disease increased, primarily with radiotherapy. Similarly, for men with LE less than 5 years, the use of radiotherapy for high-risk disease significantly rose over the study period, highlighting continued overtreatment in this population.
