@DailyInvestorSA If this were a company, the CEO would be fired. Yet in government, failure is rewarded, accountability is avoided, and the consequences are borne by constituents.
@jsteenhuisen Employees rarely act this way in isolation. Such behaviour is often a reflection of the culture set by leadership. Perhaps that is why the consequence is merely an apology, rather than meaningful disciplinary action for conduct that has brought the department into disrepute.
