Adding "mcp__x-prod-db__query" to allow permissions does fix it, but this wasn't needed before with auto-mode

I get a lot of rejections i did not get before for my read-only postgres db mcp: blocked schema enumeration, blocked for PII
Even the update-config skill is blocked

I see now our kb problem is of two types: workflows (what people actually do) and platform state (datasets evolved over time - some in Notion, some outdated, some only in the code and people's heads)
