We tackled one of the most delicate balancing acts in minimally invasive gynecology: excising endometriomas without compromising fertility.

Using a Bayesian Network Meta-Analysis (11 RCTs, n=956), we didn’t just pool the data, we probabilistically ranked the surgical options.

The SUCRA rankings revealed:
- Ovarian Reserve (AMH): Cystectomy with ultrasonic scalpel took the top spot.
- Pregnancy: Drainage with CO₂ laser vaporization ranked highest at 12 months.
- Caution: Cystectomy with suturing was the least favorable for preserving AMH.

Countless hours coding these Bayesian NMA in R definitely paid off to help map out this clinical roadmap. 🦅

Direct Aspiration (DA) or Stent Retriever (SR) for acute BAO? 🩸

The usual answer in the neuro-suite is "operator preference". But when you pool 1,851 patients across 14 studies, a clear safety gap emerges.

Here is the bottom line from our updated Meta-Analysis:

- DA takes the lead. It showed a significantly lower risk of symptomatic intracranial hemorrhage (sICH).

- A dead heat. No significant difference in 90-day functional outcomes or mortality.

This is the hard safety data you want in your back pocket when selecting your first-line device.

So, where does GAE actually fit in our current algorithm? 🤔

Despite the uncertainties, it fills a very real clinical gap. For patients who have failed conservative management, but either aren't candidates for or want to defer a TKA, GAE offers a viable, minimally invasive bridge.

Moving forward, future trials must move beyond just treating pain. We need to focus on imaging-based inflammatory phenotyping and standardizing our embolic protocols to figure out exactly who benefits most.
