I spent few days recently to completely restructure agents.md across monorepo. Now it only takes 5% of context (for 1.5 mln of lines project), but agents can deliver complex features independently. Each semantic boundary has own .md, root level only has pointers and general context

I compiled myself a plugin from several similar projects (all referenced in readme) to make sure each change made by claude gets proper self review and review by team of codex agents.
