This makes me sad. When I started at DLS in 2009 the first thing I did was read his book to get some background and history on the school and it's traditions. When the game stands tall was phenomenally written and I felt like I knew some of the people I was just around for the first time.
I never met him.

@DrDiGiorgio It all goes back to the flawed system of budget neutrality. They keep doctors in their respective tribes fighting for their piece of the pie that is only shrinking.
We’re trying to coach them up in all phases. You will be a person a lot longer than a baseball player. This is special with the boys doing this on their own volition. @NorCalBaseball

🚨 REPORT: 82% of physicians are now employed by hospitals or corporate entities. Nearly two-thirds of practices are no longer physician-owned.
This outcome should not surprise anyone.
For more than a decade, Medicare payment cuts, mounting administrative burden, growing insurer oligopolies, and policies that disadvantage independent practices have made this outcome entirely predictable.
If lawmakers want more competition, lower costs, and better access to care, preserving independent practice must be part of the conversation.
No single reform will reverse decades of consolidation. But policymakers can start by fixing Medicare, fixing prior authorization, expanding physician-owned hospitals, and strengthening antitrust enforcement.

Disagree that there's anything OK with a $7500 deductible on top of the 25K cost for an annual plan.
That defines functionally uninsured for a lot of people.
This is true and also laughable that this has been allowed to evolve. When the deductible game really started to accelerate it was all about the patients having "skin in the game" and therefore not wasting healthcare resources.
What has evolved has been a nation of functionally uninsured in which doctors are chasing after the patients deductibles rather than the insurance companies they contract with.
Talking to independent physicians, it's obvious that the big insurance carriers are doing to them, what their PBMs are doing to independent pharmacies.
They deny, underpay, slow pay, clawback, and create administrative mazes, knowing their victims don't have the time or resources to fight.
Why ? By putting financial pressures on physicians and pharmacies, it makes them more likely to sell their businesses to them , close their doors, or refer the business to their captive pharmacy or provider. All benefitting the biggest insurance companies
We need to ditch the concept of "claims" and make every delivery of medications or care as a billable event that must, by law, be paid on a timely basis , with interest charges for any delays. If the physician or pharmacy doesn't deliver , the carrier has plenty of legal options already. As does the patient.
This is not an efficient market. This is the big guy abusing the little guy. It needs to change to better the care we get in this country
