Third one in the #Clinicalpearlsinoncology series. Today let's understand pseudo-AKI in Oncology, which is a very common yet under-discussed issue. I'm using a couple of cases I came across in my practice to walk us through the concept.

Not all metastatic prostate cancers with a normal PSA are androgen receptor (AR)-independent.
Recently reflected on two patients with metastatic high-grade prostate carcinoma and disproportionately LOW / NORMAL PSA levels—one with neuroendocrine differentiation and the other without.
Both achieved >6 years of progression-free survival following initial platinum-based chemotherapy, radiotherapy to the primary prostatic mass, and CONTINUED ADT.
Take-home message:
AR signaling may remain an important driver even in PSA-low/normal aggressive variant prostate cancer.
Sometimes the biology is more hormone-sensitive than the PSA would have us believe!

A patient presents with breathlessness shortly after Cycle 1 of dual immune checkpoint inhibitor therapy.
Troponin-T is positive.
The immediate reflex may be ACS.
Pause and consider another important differential:
Myocarditis + myositis + myasthenia gravis overlap syndrome, a rare but potentially life-threatening immune-related adverse event.
Early recognition can be critical.

Is PDL1 negativity a poor prognostic marker in TNBC?
In KN 522- Path CR rates appear to be significantly lower in PDL1 negative population as against PDL1 positive patients, both in chemotherapy only arm and Pembro arm.
