@GreggWStone We use Radial Pigtail in LV connected to suction syringe to unload it during stenting .We place two wires across lesion one with Ballon and another with Stent waiting in ascending Aorta to be quickly positioned after Balloon removal . Speed is the key .

@ShariqShamimMD In such poor LV pts, I simultaneously monitor Ao and PA pressures. If SBP is more than double the PASP, I m comfortable. Whenever gap between the SBP and PASP traces start to narrow, I pause, give Intra Pul NTG 100ucg, & Sytemic NE. Once the gap widens, I resume the case.
