The FDA just approved the first oral PCSK9 inhibitor. Enlicitide (Lipfendra) — once-daily tablet — 56-60% LDL reduction in CORALreef phase 3. Numerically identical to evolocumab and alirocumab. Thread on what this means in practice.

The adherence question is the most underappreciated issue here. Injectables have near-perfect persistence. A daily tablet — fasting, 30 min before food — does not. This will matter.

Researchers just found gum disease bacteria inside calcified aortic valves.
P. gingivalis, a bacteria at the centre of periodontal disease, was found concentrated in valves removed during surgery for aortic valve stenosis.
This is new. And the story goes deeper than the headline.

A direct ask to researchers:
Future studies on periodontal disease and cardiovascular outcomes should measure Lipoprotein(a) levels.
The 1 in 5 who carry elevated levels may be a meaningfully distinct subgroup. Right now we have no data on them in this context.
