🚀 BSG EndoBridge Series (7/15): Neoplasia in Inflammatory Bowel Disease

Key messages:
🎯 Control inflammation — cumulative inflammatory burden is a major driver of colorectal neoplasia
🔍 High-quality surveillance matters: HD imaging, chromoendoscopy and meticulous mucosal inspection
✂️ Visible, well-demarcated dysplasia should be considered for complete endoscopic resection
📸 Characterise lesions systematically using the SCENIC framework
🏥 Complex, non-polypoid, fibrotic or poorly lifting lesions should be referred early to expert centres
🤝 Colectomy is no longer the default—but remains essential for selected patients with invisible, multifocal, unresectable or high-grade dysplasia

🌟 How confident are you at diagnosing, classifying and managing perianal Crohn's disease? 🌟

👬 pCD affects 20% of people with Crohn's disease, and describes fistulising and non-fistulising manifestations
🏥 It is associated with substantial symptom burden and psychosocial sequelae
✅ It has been historically under-researched. Recent global collaborative initiatives have begun to address critical knowledge gaps in this under-served IBD cohort

This fantastic review summarises recent advances in the field, offering gastroenterologists and surgeons an up-to-date overview of the evolving pCD landscape.

BIG NEWS - the largest-ever surveys of GI & colorectal trainees in the UK is in — and the verdict is clear:

❌ Training is not fit for purpose
📉 Overloaded with service work
😩 Endoscopy access is limited
💤 Out-of-hours training nearly non-existent
🤯 Trainees working on days off to meet targets

👩‍⚕️👨‍⚕️ Future consultants are worried — and many plan to go LTFT.

🧾 New in Frontline Gastroenterology:

This is one of those great reviews which FG is renowned for!

Communicating risks associated with IBD therapies is one of the most challenging – yet vital – aspects of clinical care.

This high-quality review explores:
🔹 How patient perceptions of therapy risks often differ from clinical evidence
🔹 Why undertreated IBD may pose greater dangers than treatment itself
🔹 The role of shared decision-making, visual tools & predictive models in bridging the gap
🔹 Special considerations across populations – older adults, reproductive health, prior malignancy

💡 A must-read for clinicians aiming to improve patient engagement and outcomes through clear, evidence-based dialogue and decision aids.

Patients with IBD have a 57% increased risk of venous thromboembolism (VTE) within 30 days post-discharge (OR 1.57).
🛌 Risk after non-surgical admissions even higher (OR 1.70).
💉Should extended thromboprophylaxis be routine?
