Doxycycline treats pneumonic plague.
This is not the Dark ages.
Yes. Doxycycline is an FDA-approved treatment for plague, including pneumonic plague, but it is usually an alternative rather than the preferred first-line drug for severe disease.
CDC lists doxycycline among the antimicrobials approved for plague treatment and post-exposure prophylaxis, along with streptomycin, ciprofloxacin, levofloxacin, and moxifloxacin. The adult dose used for treatment is a 200 mg loading dose, then 100 mg every 12 hours, given IV or by mouth, typically for 10–14 days or until two days after the fever ends. It was effective compared with gentamicin in a randomized trial of plague in Tanzania, and IV doxycycline improved survival in animal models of pneumonic plague when started after fever began.
It is bacteriostatic, not bactericidal. For that reason, CDC and related guidance put bactericidal agents first for pneumonic or septicemic plague: fluoroquinolones (ciprofloxacin, levofloxacin, moxifloxacin) and aminoglycosides (gentamicin or streptomycin). Doxycycline is an accepted alternative, and it is widely used for prophylaxis of people exposed to pneumonic plague. In naturally occurring severe pneumonic plague, dual therapy from two different antibiotic classes is often recommended.
