✅Normal fat deposits in the cavernous sinuses.
💡Seen in 20–27% of patients on CT. They may be more prominent in patients with obesity or Cushing syndrome. When the appearance is characteristic, no MRI or follow-up is needed.

Metastatic disase is the most common intra axial posterior fossa mass in adults. Think about hemangioblastoma if flow voids are present. #Radpearls#radiology#neuro

1/We use to think of diffusion restriction in the settings of stroke and in the settings of stroke largely ( there are some exceptions) whatever area has true diffusion restriction is dead brain. So what diffusion restricts is dead or is going to die. .
2/BUT when you look at metabolic causes of cytotoxic edema, for instance CLOCS, there its not as clear if this is irreversible and the prognosis is better than you would otherwise guess it would be.

I’ve been turning this thread over in my head for a few days.
A radiologist identified a cystic pancreatic lesion on a CT obtained for heart surgery planning and recommended further imaging. As per the imaging provided, this is a non-acute and relatively low risk finding, but did need imaging follow-up. According to the account, the patient never received that information. Two years later, she had metastatic pancreatic cancer.
As described, the radiologist appropriately identified the finding and recommended further evaluation. But that recommendation never translated into follow-up. Understanding that gap matters.
A few thoughts:
1. Low risk does not mean no risk. The finding looked low-risk at the time. The later outcome doesn’t retroactively make it an obvious cancer. But “probably benign” and “safely ignored” are different conclusions.
2. Nonurgent does not mean unimportant. A possible pancreatic cyst may not require interrupting a surgeon in the operating room. It can still require further evaluation after the immediate surgical issue is addressed.
3. The report itself is communication. Calling about every low-risk incidental finding isn’t practical or desirable. The ordering clinician has a crucial role: reading the report, discussing actionable findings with the patient, and arranging appropriate follow-up. If another clinician will manage it, responsibility needs to be explicitly handed over and accepted.
4. Radiologists must make recommendations judiciously. If every harmless incidental finding earns another scan “just to be safe,” we create anxiety, expense, and noise that can obscure meaningful recommendations. We should clearly explain what needs follow-up, why, and when.
Referrers need to be able to trust our recommendations. Patients need to be able to trust that those recommendations will reach them and receive appropriate attention.
Ovarian masses cause thinning of the ovarian tissue; smooth or eccentric thickening of ovarian tissue around a mass is suspicious for torsion.

My advice to students who are worried about choosing Radiology because of AI is simple: Don’t choose or reject a specialty based on the fear of what AI might do. Understand how the specialty itself is evolving.
I have been a radiologist for many years, and I actually feel that AI may create an interesting paradox for our profession.
Yes, AI will certainly automate many routine aspects of reporting. Some of the work that radiologists do today may not require the same human involvement tomorrow. We should acknowledge that rather than pretend nothing will change.
But there is another side to this story.
When imaging becomes faster, cheaper and more accurate, its applications are likely to expand.
We have already seen how the increasing availability of CT and MRI has transformed clinical medicine. Imagine what happens when portable MRI, affordable imaging systems, AI-assisted diagnostics and predictive imaging become more widely available.
We may end up performing far more scans, using imaging for many more clinical decisions, and detecting diseases much earlier than we do today.
And radiologists themselves will evolve from simply interpreting images to becoming more involved in clinical decision-making, interventions, imaging biomarkers and predicting disease outcomes.
So here is the paradox: AI may reduce the amount of human work required per scan, but simultaneously increase the overall importance and reach of radiology.
My advice is that if you genuinely love anatomy, imaging, technology, clinical problem-solving and the diagnostic side of medicine, Radiology remains a very exciting career choice.
However, enter it with the mindset that you will need to continuously learn, develop clinical expertise and work alongside AI rather than compete with it.
Nobody can predict the job market 15 or 20 years from now with certainty, in radiology or in any other specialty.
Don’t choose Radiology assuming it will remain exactly as it is today. And don’t reject it assuming AI will make radiologists irrelevant.
The future probably lies somewhere in between, and personally, I am quite optimistic about it. #radiology #medx #MedTwitter #ArtificialIntelligence
