Rojas BS, Procianoy RS, de Souza ACM, et al. Predicting extubation failure in neonates: The role of lung ultrasound and corrected gestational age in safe weaning in the NICU. Eur J Pediatr. 2025;184(2):144. Published 2025 Jan 17. (3/3)
Observational study by Rojas et al. using lung ultrasound scores (LUS) to predict extubation failure in 55 neonates showed no difference in scores between weaning success and failure groups. (1/3)

Sub-analysis of 33 infants born >=28w GA showed LUS to be the sole independent predictor of weaning failure! Each 1-point increase in score was associated with a 13% increase in risk of failure. (2/3)

Ever wonder how to distinguish a pneumothorax from a pneumomediastinum with #lung POCUS?
Sagaser et al. uses a few cases with videos to describe what to look for! (1/4)

Prospective observational study by Sartorius et al. showing a significant change in extended lung ultrasound scores (eLUS) as a marker of aeration and oxygenation when increasing CPAP from 4 to 6 to 8 cmH20 in infants with RDS and TTN (1/3).
#pocus#nicu
Interestingly the increase from CPAP 6 to 8 in the 30 RDS infants only showed a small absolute improvement in eLUS and the authors suggest this could mean increasing CPAP from 6 to 8 may not have clinically meaningful effects (2/3).
Check out this lung #pocus study by @FaithMyersMD et al. that investigated if a LUS within 1 week of CPAP discontinuation could predict successful weaning. A LUS <= 6 gave 100% sensitivity and <= 3 gave 100% specificity for predicting successful weaning (1/2)
