I had a friend.

Why mannitol is generally avoided in intracranial hemorrhage (ICH):
Mannitol is an osmotic diuretic that lowers intracranial pressure (ICP) by creating an osmotic gradient across the blood–brain barrier (BBB). In ICH, this mechanism can be harmful for several reasons:
1. Disrupted blood–brain barrier
In hemorrhagic stroke, the BBB is often damaged.
Mannitol can leak into brain parenchyma, abolishing the osmotic gradient.
This may lead to rebound cerebral edema, worsening ICP.
2. Expansion of hemorrhage
Mannitol causes plasma volume expansion initially and reduces blood viscosity.
This can increase cerebral blood flow and potentially promote hematoma expansion, especially early after bleeding.
3. Rebound intracranial hypertension
Accumulated mannitol in brain tissue can later draw water into the brain, causing a paradoxical rise in ICP.
4. Systemic adverse effects
Hypotension from osmotic diuresis → reduced cerebral perfusion pressure.
Electrolyte disturbances and acute kidney injury, which complicate neurocritical care.
Clinical nuance (important):
Mannitol is not absolutely contraindicated in all ICH.
It may be used temporarily as a rescue therapy for impending herniation when hypertonic saline is unavailable.
Hypertonic saline is generally preferred because it:
Restores intravascular volume
Has less rebound effect
Is safer with a disrupted BBB
Bottom line:
In ICH, mannitol can worsen cerebral edema and increase the risk of hematoma expansion due to BBB disruption and rebound effects—hence it is usually avoided in favor of hypertonic saline, except as short-term rescue therapy.
I went to a private school in Nigeria, and there wasn’t a single class where we were taught how to properly write a CV or how to ace an interview.

That’s no longer education.

And don’t say anything about I was supposed to do my own research. I paid a huge sum of amount to be in that school. I deserve my money’s worth.
