In the SOHO trial, HFOT did not reduce 28-day mortality vs standard oxygen in acute hypoxemic respiratory failure -- but it reduced intubation and improved dyspnea.
Important benefits, even without a mortality signal.

We've utterly failed across Canada to invest in hospital beds, long term care beds, community supports and homecare to meet the needs especially of older adults. Listen to Dr. Furlong explain the situation and solutions. @KaylaFurlong@CAEP_Docs
"Governments may not want data on
performance shared widely because it transparently creates winners and losers and demonstrates policy failings. Performance data can reveal crises or problems that are hard to manage and, once understood, hard to ignore."

Your patient is alert but keeps asking the same question every 2 minutes? 🧠❓ It might be Transient Global Amnesia (TGA).
The latest @CJEMonline Just the Facts by Shawn Chhabra, Achelle Cortel-LeBlanc & Miguel Cortel-LeBlanc breaks down this benign but dramatic syndrome:
ℹ️The Presentation: Sudden inability to retain new info (anterograde amnesia). Patients are oriented to self but disoriented to time and place. Common triggers include cold water immersion, physical effort, or emotional stress.
🧪The Workup: TGA is a clinical diagnosis. Most episodes last 2 to 12 hours. If you rule out focal deficits (stroke) and seizure features, acute imaging is rarely needed and often normal.
📔The Outcome: There is no increased risk of stroke or mortality after an episode. 1 in 8 patients may have a recurrence, but most return to normal baseline within a day.

How we pay for care shapes patient care. Alberta’s experience with privatized cardiac testing shows what can happen when payment models shift: testing skyrockets, but outcomes don’t necessarily improve. Unnecessary tests can lead to anxiety, false positives, and wasted resources, without actually improving patient care or outcomes.

Breakthrough original concept in #BMJ: #Guideline recommendations should consider time needed to treat. Useless or marginally beneficial #screening and advising #patients on #lifestyle can eat up huge portion of primary doc’s time, divert from useful activities.

"Real world data" is an almost meaningless phrase. Our new @bmj_latest paper pushes the more helpful phrase "routinely collected data" and explains the many limitations of such for research purposes. https://t.co/VvsxLQvhfk #Statistics
Does every ED patient need a monitor? 🩺📉
A new @CJEMonline "Just the Facts" suggests we may be over-monitoring, which directly impacts patient flow and safety.
Key Evidence:
✅ Chest Pain: If the patient is pain-free and has a normal ECG, they are safe for a non-monitored bed.
✅ Efficiency: Reducing unnecessary telemetry helps decrease EMS offload delays and nursing strain.
✅ Patient Care: Less monitoring can reduce staff alarm fatigue and patient delirium.
✅ Non-Cardiac Use: Still vital for shock, trauma, and CIWA protocols.
Use evidence-based clinical decision rules to prioritize monitors for those who truly need them and unblock the system.

Tumour lysis syndrome is a life-threatening oncological emergency with an in-hospital mortality rate ~21%. It results from rapid cell turnover & the massive release of intracellular contents like K+ & uric acid into the bloodstream (1/3)

Is your patient's pain disproportionate to their injury? 🩺🦵
@CJEMonline Just the Facts explores the diagnosis and treatment of Complex Regional Pain Syndrome (CRPS).
High Yield ED Insights:
✅ Epidemiology: Women are 4x more likely to be affected; highest prevalence is between 50 and 80 years.
✅ Diagnosis: Utilize the Budapest Criteria (Sensory, Vasomotor, Sudomotor, and Motor signs).
✅ Workup: Rule out DVT and infection; CRPS is a diagnosis of exclusion.
✅ Treatment: Start NSAIDs and gabapentinoids early to mitigate long term disability.
With diagnostic delays often reaching four years, the emergency physician's role in early detection is critical for patient recovery. 📈🚑
