@melsharabasssy @evandrofilhobr OPN is great but can lead to bad dissections. IVL would be the safest as it wouldn’t cause any tissue damage.
BTW why avoid sion wire when using OPN!?

@mandeep_mayo Asymptomatic does not mean non viability. I’m sure you have also seen many patients who restart having symptoms (that settled ) once flow is starting to be regained in their acutely occluded vessel.

Brought back afterwards and mid LAD tight stenosis predilated with 3.0 &3.5 NC up to 26atm. Then 3.5x20 drug coated balloon. No stents. Discharged on plavix + NOAC.

@evandrofilhobr @mdadnankaya Exactly. I wasn’t blaming the wire by the way. It’s just a coincidence. The perforation was managed with a 10 minute uninterrupted small balloon inflation with patient completely stable pre and post procedure.
