I had this argument with a reg a couple of days ago sha
But in unstable patients with fluid shift still occurring, 24 hours gives us a more comprehensive safety net in my opinion because for especially that 1 hour post transfusion, you could just be checking the pcv of the transfused blood
The 2020 study was quite insightful
Stating no statistical significance between then 1 hour, 4 hour and 24 hour group but then to change a narrative that’s stood for decades
A larger sample size would have been more convincing for discussions to be more profound

There’s this guy at my gym who is always nice to the plus-sized women. He’s always sweet, always complimenting, encouraging them, and all that. Now, this would be a normal thing for a nice person, except that this guy is a terrible human being.

One day, I ask him why he’s always particularly nice to the plus-sized women. He looks me dead in the eyes and says: “Bitcoin 2009.”

So, following the news, voices were raised, words were exchanged, and hearts were set ablaze.

The husband told his wife that after this ordeal she should pack out of his house, and the wife, who was very hot-headed, held her own. She maintained that she didn't know, it wasn't her fault, and he shouldn't dare talk to her that way, all whilst wailing about hearing the news that her child has sickle cell disease.

Tensions were high, and everything that could escalate was escalating. The wonderful nurse sat down, looked at the situation, and went to summon me, as per doctor should also be a relationship expert. (I'm not.)

At the time, I was handling an unstable baby. I finished up and came to assess the situation.

They both started reporting each other to me, like when kids have been summoned by an adult to talk about why they're fighting. But both their summaries at the time were that the marriage was over.

Their child was a bit unstable at the time, pale and decompensating. Myself and my colleague on call did our best and finally stabilized the child.

Whilst stabilizing the child, their pastor came in to try and talk to them about one or two things, but they were both just spewing hateful words at each other.

At this juncture, it was certainly none of my business, but I knew that if this child with sickle cell disease was raised in a broken home, his chances of survival and having a good life would be slim. So I had to sit with them and talk.

I'd spare you the overbearing details, but our discussion lasted three hours.

At the end of the day, they kind of re-said their vows to each other and agreed that they'd stay together and learn to love and raise their child with this new diagnosis with love and extra care.

What a freaking night I had!

Today is my last day of my paediatrics rotation as a house officer, and I can say I have seen things.

I have seen a lot of things.

I am not nearing paediatrics ever again.

So today, we had a kid that showed physical signs of sickle cell disease.

The father said he's AS and the mother claimed she was AA. Swore on her life and then some.

At some point, I examined this child and I was like, nah, this child has got to be SS.

The mom brought results and my colleague was like we should just test the mother and child's genotype. If the mother comes out as AA and the child SS, it'll mean it's not her child.

But this child looked like the mother. Copy and paste scenario.

So I did the blood draw from both mother and child and sent it to the lab.

And as God will have it, the mother is AS and the child SS.

TO GIVE THIS STORY SMALL CONTEXT, THE FATHER WAS ENGAGED TO A WOMAN BEFORE HER AND ENDED THE RELATIONSHIP BECAUSE THEY WERE BOTH AS, AND MARRIED HER BECAUSE SHE SAID SHE WAS AA. At least, she claimed she was.

So at this point, I'd like to tell you the ogbono became thick and the saxophone started becoming loud!

Got back from the hospital by 7pm and spent the last 5 straight hours coding and consulting for a firm that wants to market to people in Seychelles.
I hate that I have to work full time at another job whilst working full time as a doctor.
This is not how it's supposed to be.
Being a doctor should be sufficient enough to feed me and pay my bills.
I really pray things get better.
I am always tired
