🦵To Operate or Not To Operate: Degenerative Meniscal Tears
An OE Original
Degenerative meniscal tears strongly predict knee osteoarthritis and can lead to disability, time off work, and reduced quality of life.
The arthroscopic partial meniscectomy is one of the most common treatments, but results on its efficacy are mixed.
Can physical therapy be just as effective?
We did a systematic review of the literature to find out!
🏗️METHODS
We searched MEDLINE, EMBASE, and Cochrane CENTRAL for randomized controlled trials (RCTs) comparing arthroscopic partial meniscectomy and physical therapy (or exercise therapy or conservative treatment).
Outcomes of interest included:
1⃣ Pain (measured with the (KOOS pain and VAS pain scales)
2⃣ Function (measured with the IKDC and Lysholm scales).
🔎RESULTS
We found 14 articles with over 800 patients!
Pain
Pooled pain scores were significantly in favour of the physical therapy group at 6 months!
No differences between arthroscopic partial meniscectomy and physical therapy were observed at 1-year follow-up.
Function
No differences in pooled functional scores were observed between arthroscopic partial meniscectomy and physical therapy at both 6-month and 2-year follow-ups!
✅INTERPRETATION
Arthroscopic partial meniscectomy didn't provide any advantages over physical therapy for degenerative meniscal tears, and may actually result in more pain early on!
