We all later laughed about how we wished to take one or two notes, please don't judge guys we were interns earning peanuts.

@BRom970 @docakx Yhoo there is two cases I will never forget, whilst we were attending the morning an urgent call for most of to go to the ER, two guys who were shot by police after they had robbed a local tavern, the police managed to track them,they shot first and the police shot back.

❤️‍🔥 Why is Diclofenac linked with higher cardiovascular (CV) risk?

Diclofenac is basically the “most cardiotoxic NSAID” — and here’s the simple science behind it:

🔥 1️⃣ More COX-2 selective → more thrombotic risk

Diclofenac is technically a non-selective NSAID,
but functionally it behaves more like a COX-2 inhibitor.

👉 COX-2 ↓ → Prostacyclin (PGI₂) ↓
•PGI₂ normally prevents platelet aggregation
•PGI₂ dilates blood vessels
•PGI₂ protects the heart

So when PGI₂ falls, you get:
⚡ ↑ Platelet activity
⚡ ↑ Vasoconstriction
⚡ ↑ Thrombosis

This pushes the patient toward MI and stroke.

🔥 2️⃣ COX-1 (thromboxane) remains active

Platelets still produce thromboxane A₂, which promotes:
•Vasoconstriction
•Platelet aggregation

So now the balance shifts toward more clotting because PGI₂ (the protective factor) is suppressed more than thromboxane.

🔥 3️⃣ Raises BP + increases fluid retention

Diclofenac can cause:
•Sodium retention
•Fluid overload
•Increased blood pressure

→ All of these stress the heart and worsen underlying CAD.
