Keep HCQ Blood Levels Up to Avoid Cardiovascular Disease in Lupus
People taking hydroxychloroquine (HCQ) for systemic lupus erythematosus (SLE) showed markedly increased risk for atherosclerotic cardiovascular disease when blood levels of the drug were low and when long-term adherence was poor, a prospective study showed.

Netherlands pragmatic open-label RCT from 8 rheum centres tested T2T vs Sx Rx in 308 Gout pts starting ULT (87% male; 66 yrs). Remission (no flares & pain < 2) betw 18-24 mos was more in T2T pts (39% vs 24%; p=0.02), but more AE w/ Sx driven Rx (53 vs 42%) https://t.co/3tMQENl128

New paper in A&R introduces the ALARN score: a simple, practical tool for predicting mortality in antisynthetase syndrome. Based on 5 readily available clinical parameters (age, LDH, albumin, respiratory failure, NLR), the model showed excellent performance for 1- to 10-year mortality risk stratification

🦴 Spinal ligaments in #SpA: can ultrasound help?
This study developed and validated a landmark-based US protocol for thoracolumbar ligaments:
✅ Lateral costotransverse ligament
✅ Supraspinous ligament
✅ Iliolumbar ligament
In healthy controls, visualisation was feasible and reliability was moderate to high.
In patients with SpA, US detected inflammatory and structural entheseal lesions.
A new window for assessing axial inflammation beyond conventional imaging.

🆕 Postpartum disease activity in rheumatoid arthritis and spondyloarthritis: insights from the GR2 prospective cohort
The postpartum period remains a critical yet often under-characterised phase in the management of inflammatory rheumatic diseases. This analysis provides important real-world data on disease activity, treatment patterns, and breastfeeding in patients with rheumatoid arthritis (RA) and spondyloarthritis (SpA).
🔍 Key findings:
RA: Higher flare rates in the first 6 months postpartum vs pregnancy (62% vs 43%)
SpA: Stable disease activity from pregnancy to postpartum
Breastfeeding: No significant impact on disease activity in either condition
Treatment patterns:
~50% of patients (re)started bDMARDs postpartum
Continuation of bDMARDs during pregnancy was more frequent in SpA
💡 These data reinforce that the early postpartum period is a high-risk window for RA flares, while SpA appears more stable, highlighting the need for disease-specific monitoring and treatment strategies.
🤱 Importantly, breastfeeding does not seem to adversely affect disease control, supporting shared decision-making with patients.
📊 A valuable contribution to improving counselling and management across the pregnancy–postpartum continuum in rheumatology
