Marek Trněný shares primary results from the phase II CHEPA trial evaluating BV-CHEP in patients with newly diagnosed CD30+ PTCL eligible for auto-HSCT, with integrated exploratory ctDNA analysis (N = 33).
The primary endpoint was met, with 76% of patients achieving CMR at EoT; the ORR was 91% across the full cohort. At a median follow-up of 29.6 months, 2-year PFS and OS were 78% and 90%, respectively. Grade 3–4 AEs occurred in 81.8% of patients are were largely hematological: neutropenia (78.8%), thrombocytopenia (18.2%), febrile neutropenia (15.2%), and anemia (15.2%). No Grade 5 events were reported. Baseline ctDNA load correlated with TMTV (r = 0.68; p = 0.00086) and PIT (p = 0.04395). Compared with FFPE tumor tissue, plasma detected more TCR clones (87% vs 77%) and tumor-specific mutations (91% vs 81%); combined plasma and FFPE approaches enabled MRD monitoring in 100% of cases.
