two things can coexist. tourette’s involves involuntary tics. and hearing a slur like that, directed at two black men on stage, is deeply triggering. black people are allowed to feel hurt. understanding the cause doesn’t erase the impact of something so deeply damaging.
My brother had a friend at uni who had Tourettes. He would shout CUNT at his Granny and PAEDO at his Grandad. He'd shout SMELLY FANNY at girls when just trying to chat and yes, he'd shout homophobic and racist terms at gay men and people of colour. He didn't mean any of it.
