Did you know that US hospitals tie nearly a third of their Medicare reimbursement to patient-satisfaction scores — surveys measuring how happy a patient felt, NOT their outcomes?
When physicians are graded on satisfaction rather than competence, we start ordering extra tests, writing extra prescriptions… doing whatever it takes to keep the score high — even when the science says stop.
Why?
It's why my TEDx Talk went viral here on X; It's Fear.
It's the fear of being written up, losing our jobs and reputations, merely for protecting medical integrity. That fear drives appeasement over ethics, and it increases the risk of real harm; the kind that betrays the very oath we physicians swore to Do No Harm.
It’s an impossible equation: protect your integrity, or protect your income.
And that’s how the metrics meant to measure care start bastardizing it.
We built a healthcare culture that rewards perception over clinical reality, where being liked matters more than being right. And that distortion doesn’t just hurt patients — it’s breaking the very people trained to heal.
Physicians don’t burn out because they stop caring. They burn out because they care deeply in a system that punishes integrity and rewards appeasement.
