I have debriefed PAs as a GP & I shocked that after 2yrs training they are allowed to see undifferentiated pts in primary care, with poor supervision, even remote in some cases.
They are well out of their depth despite what you say.
I am not anon. There is no dishonesty in my claim & experience.
Local practices are using them as GP replacements. Partners have admitted they wish they could employ a GP, but opt for a PA as they are “cheaper”. This is cost-cutting healthcare - own it for what it is!!
Thanks to Dr Dan Poulter MP for securing last night’s important adjournment debate on Physician Associates (PAs), and for outlining serious patient safety concerns over the expansion of these roles.
This letter has no force or effect.
Platitudes without effective action don’t address doctors’ concerns to protect patients.
Its time to;
🔹reverse the name changes
🔸nationally define a restrictive scope
🔹ban their use on doctors’ rotas
🔸regulate them elsewhere to the GMC
Don’t ever tell me- change can’t be brought

Still waiting on the @rcgp to comment & clarify as to why they have been promoting the use of Locum PAs as GP replacements under the ARRS scheme.

I'm afraid I don't buy this for a moment. These are hollow words when PAs are, right now, openly being used to replace doctors on on-call rotas (& other settings). Actions are what matter, not words. What do these words actually mean when PAs are, literally, replacing doctors?
With regards to the PA issue….
My prediction for 2024. (Still
pending an EGM & RCP guidance on scope)
We will have clear statements &
guidance from NHSE, RCP, and perhaps even GMc, stating that: “PAs must not replace Doctors”
However, senior clinicians running services, short staffed, on tight budgets will choose PAs over Drs (trainees, LEDs, SAS etc) as an easy way out of a problem.
Ultimately we will all have to hold senior clinicians & trusts to account over the decisions they make.
I spent half an hour with a distraught patient today. They waited 9 months for OPA to be seen by a PA. PA couldn’t understand the recent scan result and the patient was no further forward in their treatment and symptoms not addressed. The NHS is being diluted all around us.
Very welcome ⬇️ but who’s going to police making Primary Care retreat from having ARRS PAs seeing unselected patients with minimal/remote/delayed supervision, which surely is almost by definition using them as doctor substitutes?

I REALLY think the public should ask who they are seeing
IF its a Physician Associate?
Who is supervising them & how close they are for advice/help etc?
This is-categorically-a patient safety issue
I say so as one of the national #Diabetes leads
So @RishiSunak is the reason the government has wasted £2billion more than it needed to by prolonging the strikes.
